POSITION SUMMARY:

The Case Manager/Discharge Coordinator provides case management to all inpatients/day hospital patients under general direction of the Director of Clinical Services. The Case Manager/Discharge Coordinator is a member of the multidisciplinary treatment team. Duties may vary somewhat depending on experience and areas of specialization, however, the listed duties and responsibilities are characteristic of the position. 

• Participates as an integral professional member of a multidisicplinary treatment team led by a psychiatrist.
• Facilitates the therapeutic milieu on the treatment unit.
• Receives notification of admission of patients and referrals.
• Performs case management interventions as directed by the treatment team, the treating psychiatrist and/or the Lead Social Work.
• Performs discharge planning coordination from the day of admission.
• Meets with family and patient to address family strengths and weaknesses as the pertain to discharge planning.

QUALIFICATIONS:

• Bachelor of Social Work degree from a school of Social Work accredited by the Council of Social Work Education or similar required. 
• Master of Social Work degree from a school of Social Work accredited by the Council of Social Work Education or Masters of Psychology preferred.
• Current License in the State of Michigan as a licensed Social Worker or LPC, LLPC preferred.
• Experience in an inpatient psychiatric unit preferred.
