UCP Oregon is seeking a “Bilingual Case Manager - Personal Agent Liaison ” who will work with both Spanish and English-speaking customers in our intellectual/developmental disabilities support services brokerage, which is known as UCP Connections. The Bilingual Personal Agent Liaison position is considered a leadership role within the organization. The duties of this role could include joining state workgroups, customer outreach, and other opportunities for community engagement.
As a Bilingual Personal Agent Liaison, you’ll work with 32 adults on your caseload, and it will be comprised of up to 20 Spanish-speaking individuals and their families. Our customers live in Multnomah and Clackamas Counties, and they reflect the diverse fabric of these communities. People experience disability across class, race, gender, and ethnic identities. Some of the people we serve live with their families or friends, others experience homelessness, and others live in their own homes and apartments. UCP Connections works with people who experience a wide array of disabilities and some may also experience mental health conditions and addictions, or be youth transitioning into adulthood.
You’ll assist your customers (and their families) to:
You will advocate with and on behalf of the people you work with. You will provide problem-solving (sometimes on an urgent basis) for your caseload. You will also manage intensive paperwork, including plan development, billing, caseload files, and correspondence. Finally, you’ll monitor the quality of services and ensure choice and independence.
